Zusammenfassung

Die gastroösophageale Refluxkrankheit (GERD) ist eine häufige Krankheit. Sie ist definiert durch belästigende Symptome und/oder Läsionen in der Speiseröhre als Folge von Reflux von Mageninhalt in den Ösophagus. Da es keinen diagnostischen Goldstandard gibt, kann die sichere Diagnose im Einzelfall schwierig sein. Anamnese, Endoskopie und Impedanz-pH-Metrie sind für die Beurteilung des Refluxes und seiner Auswirkungen von Relevanz, die hochauflösende Manometrie wird zur Beurteilung der pathophysiologischen Komponenten und zur Abgrenzung von Differenzialdiagnosen eingesetzt. Protonenpumpeninhibitoren (PPI) sind die Medikamente der ersten Wahl. Bei unzureichendem Effekt erfolgt eine Optimierung der PPI-Therapie, zudem stehen je nach Befundlage Kombinationsbehandlungen zur Verfügung. Bei Therapieresistenz, Unverträglichkeit von PPI oder Patientenwunsch kann in entsprechend erfahrenen Zentren eine individualisierte interventionelle Therapie auf Basis pathophysiologischer Daten erfolgen.
