Geburtshilfliche Probleme bei schwerer Adipositas

Zusammenfassung

Hintergrund

Die morbide Adipositas nimmt in allen Ländern der westlichen Welt epidemisch zu. In den USA ist die Inzidenz der Adipositas bei Frauen im reproduktiven Alter seit 1991 um bis zu 70 % gestiegen.

Ziel der Arbeit

Ursachen und epidemiologische Daten der Adipositas sollen kurz zusammengefasst werden. Die Auswirkungen der Adipositas auf die geburtshilfliche Situation und insbesondere auf das operative Management im Rahmen der Sectio caesarea werden beschrieben.

Ergebnisse

Die epidemiologischen Entwicklungen haben Auswirkungen auf die geburtshilfliche Situation – auch in Deutschland. Die Adipositas der Mutter hat Einfluss auf die Gesundheit der geborenen Kinder; sie bewirkt eine lebenslange Kompromittierung der metabolischen Situation. Zudem hat sie direkten Einfluss auf das geburtshilfliche Management im Kreißsaal. Morbid adipöse Schwangere mit einem Body-Mass-Index >30 kg/m2 weisen ein deutlich erhöhtes Risiko für Gestationsdiabetes, schwangerschaftsinduzierten Hypertonus, Präeklampsie, fetale Makrosomie, fetale Malformationen, protrahierte Eröffnungsperioden, operative vaginale Entbindungen, Schulterdystokien, notfallmäßige Sectio caesarea, postpartal erhöhten Blutverlust, Thrombosen, Thromboembolien und mütterliche Todesfälle auf. Die Sectiorate beträgt bei morbid adipösen Schwangeren 48 %.

Schlussfolgerung

Geburtshilfliche Abteilungen müssen eine „Adipositasstrategie“ entwickeln, um fatale Verläufe zu vermeiden. Dazu gehört auch die geplante elektive Sectio caesarea. Allerdings ist auch diese technisch anspruchsvoll und im postoperativen Verlauf schwierig.
