22. Gedächtnisstörungen nach Strahlen- und Chemotherapie

Zusammenfassung

Gedächtnisbeeinträchtigungen nach onkologischen Therapien werden auf gezielte Nachfrage von bis zu 50% der Patienten berichtet. Sie können mit weiteren neuro-kognitiven Beschwerden wie Lern- und Aufmerksamkeitsstörungen oder Beeinträchtigungen der Informationsverarbeitung assoziiert sein und sind eine wichtige Determinante der Lebensqualität der betroffenen Patienten. Im onkologischen Setting sind zwei Arten von Gedächtnisstörungen zu unterscheiden: Gedächtnisbeeinträchtigungen, die durch direkte therapieinduzierte Veränderungen der neuronalen Aktivität hervorgerufen werden, und Gedächtnisbeeinträchtigungen, die durch den Einfluss der Tumorerkrankung und/oder -therapie auf psychoaffektive Zustände ausgelöst werden. Während neurokognitive Defizite nach kranieller Strahlentherapie seit langem bekannt sind, werden neurokognitive Defizite nach Chemotherapie seit Mitte der 1990er Jahre intensiver untersucht.
