Endoskopische Aspekte beim geriatrischen Patienten

Zusammenfassung

Aufgrund der steigenden Inzidenz vieler Erkrankungen mit zunehmendem Lebensalter ist man im klinischen Alltag mit den Aspekten geriatrischer Patienten/-innen in der Endoskopie konfrontiert. Bei der Planung und Durchführung endoskopischer Untersuchungen und Interventionen mit entsprechenden periprozeduralen Risiken sowie bei der Sedierung der Patienten/-innen sind immer die Koerkrankungen und Komedikationen zu berücksichtigen. Über das erhöhte Patientenalter als alleiniger Risikofaktoren hinaus ist das Assessment der Komorbiditäten, der Versorgungssituation und der Komedikation insbesondere einer möglichen Antikoagulation im klinischen Alltag von grundlegender Bedeutung, um die Pathophysiologie der Erkrankung zu verstehen und die Indikationsstellung sowie Risikokonstellation endoskopischer Interventionen und der Sedierung richtig beurteilen zu können. Dabei werden endoskopische Eingriffe mit erhöhtem Blutungsrisiko von Prozeduren mit geringem Blutungsrisiko unterschieden. Diese Unterscheidung ist vom Alter unabhängig, muss aber stets mit dem individuellen thromboembolischen Risiko vor dem Hintergrund bestehender Komorbiditäten evaluiert werden. Der Indikationsstellung und Aufklärung über endoskopische Untersuchungen und Interventionen kommt bei Patienten/-innen im höheren Alter bzw. in der geriatrischen Komplexsituation eine zentrale Bedeutung zu.
