Zusammenfassung

In Industriestaaten sind die peripartale Hämorrhagie, thrombembolische Ereignisse und hypertensive Schwangerschaftserkrankungen die häufigsten Schwangerschaftskomplikationen. Sie stellen eine bedeutende Herausforderung an das interdisziplinäre Team aus Gynäkologie und Anästhesiologie. Aufgrund der Fulminanz unbehandelter Schwangerschaftskomplikationen sind die enge Absprache der Fachabteilungen und das Wissen um den verantwortlichen Aufgabenbereich essenziell. Bei akuten Blutungen obliegen dem Anästhesisten die Aufrechterhaltung suffizienter Kreislaufverhältnisse und das hämostaseologische Management. Thrombembolische Ereignisse bedürfen frühzeitiger Antikoagulation und zielgerichteter Diagnostik. Sowohl das fetale als auch das mütterliche Risiko muss abgewogen werden. Die hypertensiven Schwangerschaftserkrankungen haben ein sehr hohes Komplikationsrisiko. Neben der symptomatischen Behandlung auf der Intensivtherapiestation muss interdisziplinär der optimale Zeitpunkt der Entbindung, als einzig kausale Therapieoption, festgelegt werden.
