Handgelenkendoprothetik – ein Erfahrungsbericht

Hintergrund

Die Handgelenkendoprothetik stellt immer noch eine Ausnahmeindikation im Bereich der Handchirurgie dar. Sie wird insbesondere im deutschsprachigen Raum kontrovers diskutiert, wobei sie in den letzten Jahren zunehmend als eine Alternative zur Handgelenksversteifung als Ultima Ratio bei Panarthrose oder ähnlichen Destruktionen des Handgelenkes bzw. der Handwurzel akzeptiert wird.

Aktuelle Situation

Während es bei früheren Prothesendesigns der älteren Generationen häufig zu Komplikationen kam und nur niedrige Standzeiten erreicht werden konnten, hat sich dies bei korrekter Indikationsstellung bei den Prothesen der sogenannten 4. Generation gebessert. So sind derzeit Standzeiten von über 10 Jahren ohne Revisionsoperationen keine Seltenheit mehr. Eine weitere Entwicklung scheint sich im Einsatz als Hemiprothese, insbesondere bei posttraumatischen Destruktionen, abzuzeichnen. Diese Arbeit soll einen Überblick über den derzeitigen Stand und die Erfahrungen des Autors aus fast 15 Jahren Handgelenkendoprothetik geben.
