Die Erstlinientherapie des metastasierten Urothelkarzinoms

Zusammenfassung

Die Erstlinientherapie des metastasierten Urothelkarzinoms richtet sich nach der Eignung der Patienten für eine Cisplatin-haltige Chemotherapie. Sind die Patienten Cisplatin-fähig, gilt als Standardtherapie die Kombination Gemcitabin/Cisplatin (GC) oder alternativ Methotrexat, Vinblastin, Adriamycin, Cisplatin (MVAC). Die optimale Erstlinientherapie für die Cisplatin-ungeeigneten Patienten ist anhand der bis heute vorliegenden Daten unklar. Ein Kriterium für die Wahl der Therapie kann der PD-L1-Status („programmed cell death ligand 1“) des Tumors sein. Für PD-L1-negative Patienten (PD-L1 < 5 % für Atezolizumab und CPS [„combined positivity score“] <10 für Pembrolizumab) wird die Kombinationschemotherapie Gemcitabin/Carboplatin (GCa) favorisiert. Für PD-L1-positive Patienten (PD-L1 ≥5 % oder CPS ≥10) scheint eine Immuntherapie mit den für diese Indikation zugelassenen PD-1-Inhibitor Pembrolizumab oder PD-L1-Inhibitor Atezolizumab besser und v. a. länger wirksam zu sein. Zwei Phase-III-Studien zum Vergleich einer Immuntherapie alleine oder in Kombination mit Chemotherapie gegenüber der Standardchemotherapieschemata sollen die Frage nach der optimalen Erstlinientherapie weiter klären. Die bereits vorliegenden Daten einer dieser Studien deuten auf einen Vorteil der Kombination aus Immuntherapie und Chemotherapie in allen Subgruppen hin.
