Zusammenfassung

Die pädiatrische Nephrologie umspannt ein hochspezifisches medizinisches Gebiet mit zunehmend präventiver Bedeutung. Dazu tragen v. a. die vernetzenden Methoden der Früherkennung von Nieren- und Harnwegsfehlbildungen mittels intrauterinem und postnatalem Screening sowie neue funktionsdiagnostische Ansätze bei. Im Rahmen dieser Entwicklung hat sich in den letzten Jahren die Altersstruktur der kindernephrologischen Patienten gewandelt und die damit verbundenen Zugänge diagnostischer und therapeutischer Konzepte entwickelt. Dies gilt insbesondere für kongenitale Anomalien der Nieren und ableitenden Harnwege. Während Kinder mit einer angeborenen Harntransportstörung lange Zeit erst anhand von Komplikationen (Urosepsis, Hämaturie, Proteinurie oder Niereninsuffizienz) diagnostiziert werden konnten, gewinnt heute die Erkennung der angeborenen Anomalien durch die pränatale Diagnostik zunehmend an Bedeutung beim asymptomatischen Patienten, da die sich daraus ableitenden therapeutischen Konsequenzen und Möglichkeiten der prognostischen Einschätzung auf eine völlig neue Grundlage gestellt werden.
