Resektion und Transplantation bei hepatozellulärem Karzinom und intrahepatischem Cholangiokarzinom

Zusammenfassung

Klinisches Problem

In der Therapie des hepatozellulären Karzinoms (HCC) und des intrahepatischen Cholangiokarzinoms (iCCA) wurden in den letzten Jahren große Fortschritte gemacht.

Therapeutische Standardverfahren

Einen wesentlichen Bestandteil der kurativen Therapie stellt die Leberresektion und für das HCC auch die Lebertransplantation dar.

Neue Therapieverfahren

In der aktuellen S3-Leitlinie wird die Rolle des interdisziplinären Tumorboards bei der individuellen Therapieentscheidung gestärkt. Insgesamt bietet die Lebertransplantation für ausgewählte Patienten die besten Langzeitergebnisse. Durch den Einsatz minimal-invasiver Resektionstechniken kann die perioperative Morbidität der offenen Leberchirurgie signifikant gesenkt werden, so dass diese sowohl als kurativer Therapieansatz als auch im Rahmen des Bridgings zur Transplantation eingesetzt werden kann.

Leistungsfähigkeit

Die 5‑Jahres-Überlebensraten nach Lebertransplantation bei selektionierten, nichtresektablen Patienten innerhalb und außerhalb der Mailand-Kriterien beträgt mehr als 70 %, verglichen mit etwa 30 % ohne Lebertransplantation unter lokoregionärer Therapie.

Bewertung

Im folgenden Artikel wird der aktuelle Stand der evidenzbasierten chirurgischen Therapie des HCC und des iCCA anhand der Empfehlungen der aktuellen S3-Leitlinie aufgezeigt.

Empfehlung für die Praxis

Die differenzierte Lokaltherapie des HCC in Zirrhose erfolgt anhand vieler patienten- und tumorspezifischer Faktoren. Neben der chirurgischen Resektion spielt dabei die Lebertransplantation als kurative Therapie bei irresektablen HCC sowohl innerhalb als auch außerhalb der Mailand-Kriterien eine wichtige Rolle. Beim resektablen iCCA oder HCC ohne Zirrhose im nicht fernmetastasierten Stadium stellt die chirurgische Resektion die Therapie der Wahl dar.
