Zusammenfassung

Die Mitralklappeninsuffizienz ist in den westlichen Ländern die zweithäufigste Herzklappenerkrankung. Je nach Ätiologie wird zwischen primärer und sekundärer Mitralklappeninsuffizienz unterschieden. Die primären Mitralklappeninsuffizienzen können sowohl über verschiedene Zugangswege als auch mittels unterschiedlicher operativer Techniken behandelt werden. Bevorzugt werden derzeit in spezialisierten Zentren minimal-invasive Verfahren, die vergleichbar gute Ergebnisse mit den offenen chirurgischen Verfahren zeigen. Bei den operativen Techniken haben sich aktuell segelerhaltende Techniken, z. B. durch den Einsatz künstlicher Sehnenfäden, gegenüber der Resektion durchgesetzt. Die Therapie der sekundären Mitralklappeninsuffizienz erfordert eine interdisziplinäre Zusammenarbeit und erfolgt zunehmend interventionell.
