Natürlicher Verlauf, konservative Behandlungsmethoden und optimaler Operationszeitpunkt der symptomatischen Hüftgelenkdysplasie

Zusammenfassung

Hintergrund

Die Hüftdysplasie ist eine der häufigsten Fehlstellungen am Hüftgelenk und nachgewiesener Risikofaktor für eine sekundäre Arthrose.

Fragestellung

Wie beeinflusst die Hüftdysplasie den Verschleiß des Gelenks, wie und wann können Patienten konservativ geführt werden, und wann ist der bestmögliche Operationszeitpunkt?

Material und Methoden

Eine Untersuchung der bis 2021 publizierten Literatur erfolgte in Bezug auf den natürlichen Verlauf bei Hüftdysplasie sowie zur Identifizierung von Faktoren bei Auswahl geeigneter Patienten und Festlegung des richtigen Operationszeitpunkts.

Ergebnisse

Beinahe alle publizierten Studien weisen auf einen Zusammenhang von Hüftdysplasie und der Entstehung einer Arthrose hin. Die verminderte azetabuläre Überdachung und insbesondere die Faktoren Instabilität und Subluxation sind mit dem erhöhten Schadensrisiko verknüpft. Beckenosteotomien sollten vornehmlich bei Patienten ohne relevante Arthrose erfolgen. Parallel sind bei der operativen Versorgung koexistente Hüftpathologien zu behandeln.

Schlussfolgerung

Beckenosteotomien sind bei Patienten mit Beschwerden im Adoleszenten- und Erwachsenenalter die Therapie der ersten Wahl zur Behandlung der Hüftdysplasie. Eine konservative Therapie der symptomatischen Hüftdysplasie ohne wesentliche Arthroseerscheinungen ist bis zum 50. Lebensjahr nur eine begrenzt wirksame Alternative und sollte deshalb allenfalls zeitlich limitiert durchgeführt werden, um den protektiven Einfluss für das Gelenk nicht zu verspielen. Sie ist vornehmlich beim noch asymptomatischen oder bereits degenerativ veränderten Gelenk indiziert.
