Zusammenfassung

Schilddrüsenhormone haben während einer Schwangerschaft profunde Auswirkungen auf die kindliche Entwicklung und die mütterliche Gesundheit. Schilddrüsenfehlfunktionen müssen daher rechtzeitig diagnostiziert und adäquat behandelt werden. Schwangerschaftsspezifische physiologische Veränderungen führen unter anderem dazu, dass der Referenzbereich des thyreoideastimulierenden Hormons verändert ist und trimesterspezifische Grenzwerte beachtet werden müssen. Dieser Beitrag fasst die wichtigsten diagnostischen und therapeutischen Aspekte vor, während und nach einer Schwangerschaft zusammen. Bezüglich der Zeit vor der Schwangerschaft wird auf die Jodidsupplementation, die präkonzeptionelle Untersuchung der Schilddrüsenstoffwechsellage und die Bedeutung von Schilddrüsenfunktionsstörungen für die Fertilität und Erfüllung des Kinderwunschs eingegangen. Mit Blick auf die Zeit während der Schwangerschaft werden die Auswirkungen von Hypothyroxinämie, Hypothyreose und Hyperthyreose sowie die Effekte ihrer Therapie auf die Entwicklung des Kindes erörtert. Zuletzt wird beschrieben, was in der Stillzeit und bei Postpartumthyreoiditis zu beachten ist.
