51. In-vitro-Serumdiagnostik

Zusammenfassung

Die In-vitro-Allergiediagnostik umfasst Labormessverfahren von Parametern aus Körperflüssigkeiten (Serum, Plasma, Nasalsekret, Bronchialsekret etc.) und von zellulären Parametern (z. B. Bestimmung von CD63 auf basophilen Granulozyten nach Allergeninkubation oder allergenspezifische Stimulation von Lymphozyten bei Kontaktallergie). Im Kapitel In-vitro-Serumdiagnostik werden nur die in den Körperflüssigkeiten, zumeist im Serum, nachweisbaren Parameter behandelt. Zu diesen Parametern zählen die serologischen Messverfahren für allergierelevante Antikörper (Gesamt-IgE-Bestimmung, Messung von allergenspezifischem IgE und IgG oder IgG-Subklassen) und die Nachweisverfahren für zelluläre Entzündungsparameter im Serum (Tryptase, eosinophiles kationisches Protein – ECP). Für die Messung dieser Parameter stehen heute mehrere sensitive, teils vollautomatisierte Labortestverfahren zur Verfügung. Eine wissenschaftliche Bewertung aller verfügbaren Testsysteme für die In-vitro-Allergiediagnostik erfolgt regelmäßig in Leitlinien und Stellungnahmen der maßgeblichen internationalen und nationalen Fachgesellschaften.
