Zusammenfassung

Aufgrund der steigenden Anzahl an endoprothetischen Gelenkersatzoperationen bei älteren Patienten, aber auch bei Patienten mit schweren Grund- und Begleiterkrankungen steigt die Anzahl der Revisionsoperationen aufgrund von Infektionen kontinuierlich an. Die Infektion einer Endoprothese stellt ein komplexes Krankheitsbild dar. Der Beitrag gibt eine detaillierte Übersicht über die Behandlung von periprothetischen Infektionen mit Schwerpunkt auf diagnostischem Vorgehen und Therapieentscheidung. Man unterscheidet bei periprothetischen Infektionen zwischen Infekten mit reifem oder unreifem Biofilm auf der Implantatoberfläche. Abhängig davon stehen unterschiedliche Behandlungskonzepte zur Verfügung. Hochakute Infektionen stellen beim künstlichen wie auch beim Nativgelenk einen orthopädischen/chirurgischen Notfall dar. Low-grade-Infektionen spielen v. a. im Bereich der Wechselendoprothetik eine Rolle. Größtmöglicher Erfolg wird nur im Zusammenspiel von frühzeitiger Diagnosestellung, suffizientem chirurgischem Vorgehen und maßgeschneiderter antiinfektiver Therapie erzielt.
