Zusammenfassung

Hintergrund

Die global steigende Prävalenz von Adipositas und Typ-2-Diabetes macht die Entwicklung neuer pharmakologischer Therapieoptionen erforderlich, denn konservative Behandlungen, die auf eine Lebensstiländerung abzielen, sind oft nicht dauerhaft erfolgreich.

Ziel der Arbeit

Die derzeit national und international verfügbaren pharmakologischen Adipositastherapien sowie aktuelle Forschungsfortschritte sollen vorgestellt werden.

Material und Methoden

Die kürzlich zu peptidbasierten Polyagonisten und dem peptidbasierten Transport von Steroid- und Schilddrüsenhormonen publizierten Arbeiten zur künftigen Therapie von Adipositas und Diabetes werden zusammenfassend dargestellt.

Ergebnisse

Die derzeit verfügbaren pharmakologischen Adipositastherapien sind z. T. wenig effektiv und/oder sehr nebenwirkungsreich. Vielversprechend ist dagegen der neue Ansatz, unimolekulare Peptidmoleküle zu synthetisieren, die bis zu 3 Rezeptoren mit ähnlicher Affinität aktivieren. Die präklinische Erforschung eines GLP-1-/Glukagon- (GLP: „glucagon-like peptide“) und eines GLP-1/GIP-Koagonisten (GIP: „glucose-dependent insulinotropic polypeptide“) sowie eines GLP-1/GIP/Glukagon-Triagonisten ergab in verschiedenen Tiermodellen günstige Effekte bezüglich einer Senkung des Körpergewichts und des Blutzuckerspiegels. Ähnliche positive Effekte wurden für Steroid- und Schilddrüsenhormone beschrieben, die mittels eines Peptidtransportmoleküls gezielt in bestimmte Gewebe transportiert werden. Besonders vielversprechend sind hier GLP-1/Östrogen und Glukagon/T3 (Trijodthyronin).

Schlussfolgerungen

Dieser Artikel bietet eine detaillierte Zusammenfassung der momentan erhältlichen Pharmaka zur Adipositas- und Diabetestherapie sowie einen Ausblick auf die zukünftigen neuen Therapieoptionen.
