Zahl der Schocks sagt nichts über KHK-Last

Die Annahme, dass beim außerklinischen Kreislaufstillstand (OHCA) die Zahl der notwendigen Defibrillationen den Schweregrad einer zugrunde liegenden KHK widerspiegelt, trifft wohl nicht zu. Als Selektionskriterium für eine Koronarangiografie taugt sie daher nicht.

Metaanalyse spricht für Nutzen von Kortikosteroiden bei schwerer CAP

Geht es nach einer aktuellen Metaanalyse, ist die kurzzeitige Verabreichung von niedrigdosierten systemischen Kortikosteroiden bei schweren Nicht-COVID-19-Pneumonien bzw. ARDS (Akutes Atemnotsyndrom) mit Blick auf das Überleben eher vorteilhaft. Die Sicherheit der Evidenz ist allerdings ausbaufähig.

Unspezifischer und später Beginn einer Autoimmunerkrankung

Hinter Gelenkschmerzen, Gewichtsverlust und intermittierendem Fieber bei einer über 70-Jährigen steht eine schwere Erkrankung, die eigentlich eher mit einem Beginn in jüngeren Jahren und anderen Symptomen in Verbindung gebracht wird.


Was bringt Bikarbonat bei Azidämie und akutem Nierenversagen?

Ein zu niedriger pH-Wert von Intensivpatientinnen und -patienten im akuten Nierenversagen lässt sich durch die Gabe von Bikarbonat zwar anheben. Doch welchen Nutzen hat diese Maßnahme? Die BICARICU-Forschungsgruppe hat nach Antworten gesucht.
