Zusammenfassung

Viele Patienten mit einer chronisch-entzündlichen Darmerkrankung (CED) sind von chronischen Schmerzen sowie einer ausgeprägten Fatigue-Symptomatik betroffen. Auch in Phasen der Remission sind die Patienten häufig in ihrer Lebensqualität hierdurch stark eingeschränkt. Im klinischen Alltag, aber auch in der Forschung finden diese Themen bisher nur sehr unzureichend Beachtung. Die bisher durchgeführten Studien zu chronischen Schmerzen sind inhaltlich sehr heterogen. Häufig war die Zahl der eingeschlossenen Patienten sehr klein. Es kann daher bisher keine belastbare Empfehlung zur Behandlung gegeben werden. Beim Thema Fatigue ist die Situation nur wenig anders, wobei hier in den letzten Jahren insbesondere zur Entstehung der Symptomatik eine Vielzahl neuer Erkenntnisse gewonnen werden konnten. Aktuell wird angenommen, dass es sich um ein multifaktorielles Geschehen handelt. Neben der Mikrobiota, entzündlichen Veränderungen, Mangelerscheinungen sowie einer psychischen Komponente scheinen auch körperliche Aktivität, Rauchen sowie Ernährung eine zentrale Rolle zu spielen. So vielschichtig wie die zur Entstehung beitragenden Faktoren sind auch die Therapieansätze. Auch hier fehlen große kontrollierte Untersuchungen, um fundierte Therapieempfehlungen geben zu können.
