A 10-mo-old boy presented with gradual onset abdominal distension and bilious vomiting. On examination, the vitals were stable and the child had distension of the upper abdomen. Plain radiograph of the abdomen revealed gross gaseous distension of the stomach (Fig. 1). An upper GI obstruction was suspected and upper GI contrast study was performed. The contrast study revealed grossly distended stomach and first part of duodenum, and a contrast filled outpouching within the lumen of the second part of duodenum (Fig. 2) showing the classical ‘windsock’ appearance. An imaging diagnosis of intraluminal duodenal diverticulum because of duodenal web was made and the child was taken up for surgery.
