Ein 57-jähriger verheirateter, bisher immer gesunder, Manager bemerkte 2014 nach dem Duschen erstmals plötzlich einsetzende Schmerzen in beiden Ellenbogen. Im Wartezimmer des Hausarztes kam es dann kurze Zeit später zu einem Kreislaufkollaps. Trotz sofortiger Reanimationsmaßnahmen durch das Praxis-team gelang es erst dem Notarzt nach zweimaliger Defibrillation bei Kammerflimmern den Kreislauf nach etwa 30 Minuten wieder zu stabilisieren. Nach Eintreffen im Krankenhaus mussten noch weitere sechs Defibrillationen bei erneutem Kammerflimmern durchgeführt werden. In der Notfall-Koronarangiografie konnte die einen akuten Hinterwandinfarkt auslösende 1-Gefäß-Erkrankung durch eine Stent-Implantation behandelt werden. Sofort erfolgte eine leitlinienentsprechende Hypothermie und Sedierung. Nach Ausschleichen der protektiven Maßnahmen kam es dennoch zu einem nur sehr verzögerten Erwachen. Ein MRT zeigte beidseitige Thalamusläsionen und ein nicht raumforderndes frontales Falxmeningeom. Nach dilatativer Tracheotomie kam es während des protrahierten Weaning zu einer Aspirationspneumonie, als deren Folge sich eine Critical-Illness-Polyneuropathie komplizierend entwickelte. Etwa zwei Wochen nach dem Ereignis trat erstmals ein tonisch-klonischer Grand-mal-Anfall auf. Eine antiepileptische Therapie mit Levetiracetam wurde begonnen und aufgrund eines im EEG diagnostizierten nonkonvulsiven Status mit Topiramat kombiniert. …
