Revisionseingriffe beim Karpal- und Kubitaltunnelsyndrom

Zusammenfassung

Hintergrund

Die beiden häufigsten Engpasssyndrome peripherer Nerven sind das Karpaltunnelsyndrom als Einengung des N. medianus im Karpaltunnel und das Kubitaltunnelsyndrom als Einengung des N. ulnaris im Ellenbogenbereich. Der Druckschädigung des Nervens folgen Schmerzen, Hypästhesien und Parästhesien in seinem sensiblen Innervationsbereich und Paresen der von ihm innervierten Muskulatur. Nach Ausschöpfen der konservativen Therapie kann allein die operative Dekompression die mechanische Engstelle beseitigen, mit dokumentierten hohen Erfolgsraten. Die Revisionsrate aufgrund von Beschwerdepersistenz oder eines Rezidivs wird im Fall des Karpaltunnelsyndroms auf ca. 5 %, im Fall des Kubitaltunnelsyndrom auf bis zu 19 % geschätzt. Die häufigsten Gründe für das Therapieversagen der primären Dekompression sind die inkomplette Dekompression und die Ausbildung von perineuralen Narben und Adhäsionen.

Therapie

Nach gründlicher Überprüfung der Operationsindikation ist das Ziel des Revisionseingriffs die komplette Dekompression des betroffenen Nervenabschnitts mit Einbettung des Nervens in vitales Gewebe zur Wiederherstellung der Nervengleitfähigkeit, um die neuronale Regeneration zu ermöglichen und um neue Vernarbungen zu verhindern. Hierzu gehören Verfahren der Neurolyse, der Nervenumhüllung und der Nervenrekonstruktion sowie im Falle des N. medianus Lappenplastiken und im Falle des N. ulnaris Nerventranspositionen.

Ergebnisse

Insgesamt kann durch den Revisionseingriff die Beschwerdesymptomatik verbessert werden, nichtsdestotrotz ist das klinische Ergebnis schlechter als nach dem Primäreingriff und eine Beschwerdepersistenz ist häufig.
