Ketogene Ernährungstherapien: Grundlagen der Durchführung und Anwendung bei angeborenen Stoffwechselstörungen

Zusammenfassung

Ketogene Ernährungstherapien (KETh) umfassen verschiedene Ernährungsformen, denen die Reduktion der Kohlenhydratzufuhr bei gleichzeitiger Steigerung der Fettzufuhr gemeinsam ist. Unterschieden werden muss zwischen KETh als Lifestyle-Maßnahme und als medizinisch notwendige, professionell begleitete und dauerhaft aufrechterhaltene Therapie. Am häufigsten werden KETh bei pharmakoresistenten Epilepsien und Epilepsiesyndromen eingesetzt. Bei angeborenen Stoffwechselstörungen gibt es Erkrankungen mit klarem pathophysiologischen Hintergrund, für die KETh die Therapie der Wahl darstellen. Zunehmend erfolgt der Einsatz der KETh auch bei angeborenen Stoffwechselstörungen mit weniger eindeutigen pathophysiologischen Mechanismen und bei angeborenen Stoffwechselstörungen, bei denen eine pharmakoresistente Epilepsie trotz der zugrunde liegenden Stoffwechselstörung mithilfe einer KETh behandelt wird. Sowohl für pharmakoresistente Epilepsien als auch angeborene Stoffwechselstörungen sind KETh etablierte Therapieoptionen und keinesfalls „alternative“ Therapieformen.
