Piezoosteotomie in der Dysgnathiechirurgie

Zusammenfassung

Die Piezochirurgie wird seit rund 35 Jahren in der Mund‑, Kiefer- und Gesichtschirurgie eingesetzt und findet vermehrt ihre Anwendung in der Dysgnathiechirurgie. In diesem Beitrag werden zunächst die Entwicklung und der Nutzen der Piezoosteotomie in der Dysgnathiechirurgie erläutert. Dann werden die Determinanten intraoperativer Blutverlust, Operationszeit und neurosensorische Störungen nach Piezoosteotomie im Vergleich zur konventionellen Instrumentierung anhand einer Literaturrecherche dargestellt. Die Piezoosteotomie ermöglicht einen geringeren Blutverlust und eine bessere Nervenschonung intraoperativ und ergo postoperativ bei zuverlässiger Reossifikation durch kontrollierte Hitzeentwicklung.

Mit dem Semmes-Weinstein Monofilament Test wird untersucht, ob ein Patient sensible Reize auf der Haut spürt. Das Assessment hilft dabei, den Bereich einer neuronalen Schädigung abzubilden und dem Therapeuten einen Ansatz für ein spezifisches Sensibilitätstraining zu geben.
