Klumpfußtherapie nach Ponseti

Zusammenfassung

Ziel

Schmerzfreier, plantigrad aufsetzbarer, voll funktionsfähiger Fuß durch sanfte Redression ohne große Operation und mit reduzierter Rezidivwahrscheinlichkeit.

Indikationen

Idiopathischer Klumpfuß; neurogene oder sekundäre Klumpfüße.

Kontraindikationen

Keine.

Operationstechnik

Gleichzeitige Korrektur aller Komponenten des Klumpfußes bis auf Spitzfuß. Weitestgehend konservativ, mit sanfter Gipsredression. Leichte Supination, um Hohlfuß zu beheben, und physiologische Position der Fußwurzelknochen durch zunehmende Abduktion und abnehmende Supination unter Gegendruck auf dem Taluskopf. Perkutane Achillotenotomie falls Spitzfuß durch weitere Redression nicht korrigierbar. Wegen Rezidivneigung evtl. weitere Operationen notwendig, ggf. mit erneuter Gipsredression. Bei Restdeformität evtl. perkutane Achillessehnenverlängerung nach Hoke, perkutaner Plantarfaszienrelease nach Ulzibat und Tibialis-anterior-Sehnentransfer.

Weiterbehandlung

Tragen der Abduktionsschiene zur Stabilisierung der klinischen Ergebnisse in den ersten 3 Monaten immer, bis zum Ende des 3. Lebensjahres zu Nacht- und Ruhezeiten, bei zunehmender Beweglichkeit des Kindes nur noch nachts. Verlaufskontrollen alle 3–4 Monate.
