Kniegelenknahe Osteotomien bei Bandinsuffizienzen

Zusammenfassung

Hintergrund

Die ossäre Geometrie der unteren Extremität beeinflusst das Ausmaß der Kniegelenkinstabilität nach Bandrupturen. Achsfehler stellen einen signifikanten Risikofaktor für das Versagen von Bandplastiken dar. Neben der Entlastung eines arthrotisch veränderten Kompartiments haben sich daher kniegelenknahe Osteotomien in der Behandlung von Bandinstabilitäten als kombiniertes oder sogar isoliertes Therapieverfahren etabliert. In speziellen Fällen werden Osteotomien auch zur Stabilisierung eines Gelenks ohne Bandplastik eingesetzt.

Verfahren

Neben der Varus‑/Valguskorrektur hat sich auch das Konzept der Slope-Korrektur etabliert, da bereits eine geringfügige Änderung des Slope (≤5°) zu einer klinisch signifikanten Änderung der anteroposterioren Translation führt. Insbesondere bei der chronischen posterioren und v. a. posterolateralen Instabilität ist eine valgisierende und Slope-erhöhende Tibiakopfosteotomie der erste Therapieschritt und eine Bandplastik erfolgt lediglich optional. Isolierte Slope-Korrekturen sind selten, sollten aber bei Patienten mit wiederholten Transplantatrupturen nach Plastik des vorderen Kreuzbandes und Slope >12° in Erwägung gezogen werden.
