Perkutane Radiotherapie zur Schmerzlinderung bei ossären Metastasen

Zusammenfassung

Hintergrund

Ossäre Metastasen sind die häufigste Schmerzursache onkologischer Patienten.

Ziel

Ziel war, die Möglichkeiten der perkutanen Radiotherapie zur Schmerzlinderung darzulegen.

Material und Methoden

Es erfolgte eine selektive Literaturrecherche in der Datenbank PubMed.

Ergebnisse

Schmerzhafte Knochenmetastasen können mit modernen Bestrahlungstechniken effektiv und nebenwirkungsarm therapiert werden. Die Ansprechraten liegen zwischen 50 und 90 %, komplette Schmerzfreiheit erreichen zwischen 10 und 50 % der Patienten. In Bezug auf die schmerzlindernde Wirkung werden Einzeit-, hypo- und normofraktionierte Konzepte unterschieden. Gemäß aktuellen Metaanalysen ist eine Einzeitbestrahlung mit 8 Gy im Vergleich zu fraktionierten Konzepten analgetisch isoeffektiv. Nach konventioneller Fraktionierung mit 2–3 Gy ist 3–6 Monate post radiationem eine bessere Remineralisierung als nach Einzeitbestrahlung erreichbar, außerdem treten weniger Schmerzrezidive auf und sind entsprechend weniger Wiederbestrahlungen erforderlich. Die Einzeitbestrahlung mit 8 Gy wird weltweit als „standard of care“ in der Therapie symptomatischer unkomplizierter Knochenmetastasen ohne Fraktur oder Spinalkanalkompression empfohlen. Patienten in gutem Allgemeinzustand und mit einer Überlebensprognose von über 3 Monaten sollten normofraktioniert therapiert werden. Wegen mögliche vorübergehender Schmerzverstärkung wenige Tage nach der ersten Fraktion („pain flare“) ist eine optimale prätherapeutische Schmerzeinstellung sowie ggf. eine antiödematöse Prophylaxe vor Einzeitradiatio erforderlich.

Schlussfolgerung

Angesichts verschiedene radiotherapeutische Optionen zur Schmerzlinderung bei ossären Metastasen sollte das optimale Konzept interdisziplinär und gemeinsam mit dem Patienten individuell festgelegt werden.

Umfrage: Topika mit Dexpanthenol bei Radiodermatitis empfohlen

In der topischen Prävention der akuten Radiodermatitis werden Zubereitungen mit Dexpanthenol oder Harnstoff von deutschsprachigen Fachkreisen in der Radioonkologie am häufigsten empfohlen und als am wirksamsten bewertet. Bei der Behandlung der strahlenbedingten Hautschäden liegen Topika mit Dexpanthenol oder Kortikosteroide vorn [1].

Handekzem: Adhärenz bei topischer Therapie nicht zufriedenstellend

Einer klinischen Studie zufolge wendet nur etwa die Hälfte der Ekzem-Patient:innen ihre topische Therapie mit Kortikosteroiden wie verordnet an. Darüber hinaus nahm die Adhärenz im Zeitverlauf weiter ab. Bei einer gleichzeitig applizierten barrierestabilisierenden Basiscreme blieb die Anwendungsfrequenz dagegen über die Zeit stabil [1].
