Kollisionstumor des Gehirns

Tumorkomponenten aus einem Glioblastom und einem Meningeom

Zusammenfassung

Ein 51-jähriger Mann wurde wegen Kopfschmerzen, Fazialisparese und zunehmender Einschränkung des Gesichtsfeldes stationär aufgenommen. Die CT-Untersuchung ergab eine einzelne intrazerebrale kontrastmittelspeichernde Läsion im linken Frontallappen. Intraoperativ war der Tumor gut abgegrenzt. Die Gefrierschnittuntersuchung zeigte einen hochgradigen gliomatösen Tumor. In den Paraffinschnitten fanden sich zusätzlich zu der gliomatösen Komponente einige scharf abgegrenzte Nester von meningothelialen Zellen. Die Immunhistochemie mit Gliafaser-saurem Protein und epithelialem Membranantigen bestätigte den gemischten Aufbau des Tumorgewebes aus einem Glioblastom WHO-Grad-IV und einem meningothelialen Meningeom WHO-Grad-I.
Das Zusammentreffen dieser beiden Tumorkomponenten am gleichen Ort und zur gleichen Zeit führt zu der Annahme, dass der Kollisionstumor durch eine maligne Transformation einer reaktiven Gliose rund um das Meningeom, entstanden sein könnte.
