Präkanzerosen und Malignome des Gastrointestinaltrakts

Geschlechtsspezifische Unterschiede bezüglich Inzidenz, Risikoprofil, operativer Therapie und sozioökonomischen Faktoren

Zusammenfassung

Hintergrund

Geschlechtsspezifische Unterschiede erlangen zunehmende Bedeutung in der klinischen Medizin, so auch in der Onkologie.

Methode

Mittels einer narrativen Übersicht sollen geschlechtsspezifische Unterschiede von ausgewählten Karzinomen des Gastrointestinaltrakts bzw. Anhangsorganen und deren Präkanzerosen hinsichtlich patienten-, befund- und therapieassoziierter Aspekte eruiert werden.

Ergebnisse

Bei Männern scheint bis auf einige Ausnahmen die Häufigkeit der Krebsneuerkrankungen höher zu sein als bei Frauen. Die Überlebensraten fallen bei weiblichen Patienten überwiegend günstiger aus, wobei diese meist eine bessere Compliance und einen höheren Pflegeanspruch aufweisen, während bei Männern mehr stationäre Sterbefälle und häufigere Krankenhauseinweisungen zu verzeichnen sind. Bei einigen Krebsarten mit deutlicher Geschlechterpräferenz wird bereits seit längerer Zeit ein Zusammenhang mit dem Hormonstatus vermutet. Männliches Geschlecht ist neben anderen Faktoren mit einer schlechteren Prognose beim kolorektalen Karzinom (KRK) assoziiert. Im Zusammenhang mit Präkanzerosen und Risikofaktoren spielen das Geschlecht, diverse Genotypen, Polymorphismen, sozioökonomische und verhaltenspsychologische Faktoren eine Rolle.

Diskussion

Geschlechterabhängige Unterschiede lassen sich in Inzidenz, Risikoprofil, sozioökonomische Aspekte, Symptomatologie, Diagnostik, chirurgisch-operative Angehbarkeit und Versorgung mit Komplikationshäufigkeit/‑spektrum sowie Prognose klassifizieren.

Schlussfolgerung

In Zusammenschau spielen nicht nur biologische Voraussetzungen, sondern auch geschlechtsspezifische Verhaltensmuster als komplexes interagierendes Konstrukt eine durchaus mitentscheidende Rolle für die Prognose und das Langzeitüberleben bei Krebserkrankungen des Gastrointestinaltrakts.
