7. Komplikationen beim Legen und im Umgang mit Thoraxdrainagen

Zusammenfassung

Thoraxdrainagen und pleurale Punktionen stellen ein buntes medizinisches Mosaik dar, geprägt von vielfältigen Einflussgrößen: Ausbildung des Durchführenden, rechtfertigende Indikationsstellung, Art und Dauer der Intervention sowie des Punktates, Drainagemanagement, Interventionsmodus, Umgebungssetting und als wichtigsten Parameter den behandelnden Patienten. Die Art und Schwere möglicher Komplikationen sowie deren Management sind ebenso vielfältig und von den beschriebenen Einflussgrößen geprägt. Anhand dieser Übersicht kann sich jeder Therapeut ein falladaptiertes Risikoprofil und eine strukturierte Vorgehensweise erstellen, um „pit-falls“ zu vermeiden. Nur eine differenzierte und selbstkritische Würdigung von Interventionsumständen erscheint geeignet, einen Behandlungserfolg zu sichern.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
