Komplikationen in der Spätphase nach Augenverätzung

Operative Therapiestrategien bei Folgeschäden an der Augenoberfläche

Zusammenfassung

Verätzungen der Augenoberfläche können trotz adäquater Therapie in eine dauerhafte Schädigung der Augen mit Vernarbungen im Bereich der Bindehaut und damit einhergehender Lidfehlstellung mit Trichiasis und Ausbildung eines schweren, trockenen Auges münden. Schädigungen der Hornhaut führen zu Vernarbungen und chronischen Ulzerationen. Bei fortgeschrittenen Ischämien und Zerstörungen limbalen Gewebes kommt es zu einer Limbusstammzellinsuffizienz. Operative Therapien zielen auf die Korrektur von Lidfehlstellungen, die Rekonstruktion von Bindehaut und Fornix mit geeignetem Gewebeersatz und die Wiederherstellung eines stabilen, avaskulären und transparenten Hornhautepithels, ggf. durch Transplantation epithelialer Stammzellen. Fortschreitende Ulzerationen der Hornhaut, die nicht auf eine konservative Therapie ansprechen, können entsprechend der Defekttiefe durch Transplantation von Amnion oder Hornhaut bzw. durch eine Bindehautdeckung behandelt werden. Die operative Therapie dieser Patienten ist meist spezialisierten Zentren vorbehalten und erfolgt bisweilen in multidisziplinärer Kooperation.
