Zusammenfassung

Durch intrauterine oder postnatale sonographische Screeninguntersuchungen werden zunehmend häufig Dilatationen der Harnwege oder Nierenfehlbildungen entdeckt. Die wenigsten Harntraktanomalien bedürfen der unmittelbaren Therapie. Vor allem ureteropelvine Stenose und primärer Megaureter erfordern jedoch eine prognostische Einordnung zwischen einer korrekturbedürftigen Obstruktion und einer Dilatation ohne Auswirkung auf die Nierenfunktion. Trotz zunehmender Zahl von Biomarkern bleiben derzeit Sonographie, Diureseszintigraphie und/oder Magnetresonanztomographie die tragenden Säulen der weiterführenden Diagnostik. Die Verhinderung progredienter Nierenparenchymschäden ist Hauptziel diagnostischer und therapeutischer Strategien unter Vermeidung unnötiger oder unnötig belastender Interventionen.
