Neuromodulation bei primären Kopfschmerzen im Jahr 2019 – noch zeitgemäß?

Neue Daten zu invasiven und nichtinvasiven Verfahren bei Migräne und Clusterkopfschmerz

Zusammenfassung

Neuromodulierende Verfahren haben das therapeutische Arsenal für die Behandlung von primären Kopfschmerzen wie Migräne und Clusterkopfschmerz um einen methodisch neuen Ansatz bereichert. Während die invasiven Verfahren meist Patienten mit refraktären und chronischen Verläufen vorbehalten sind, bieten sich die nichtinvasiven Verfahren zunehmend auch für weniger stark betroffene Patienten mit episodischen Verläufen an. Neben einer Übersicht über aktuelle kontrollierte Studien zu den einzelnen Verfahren erfolgen eine kritische Wertung mit Empfehlung zum Einsatz der entsprechenden Verfahren im klinischen Alltag sowie eine Einordnung in die aktuellen Therapiealgorithmen. Problematisch bleibt bei den meisten Ansätzen, dass nur wenige kleine kontrollierte Studien mit teils merklichen methodischen Limitationen vorliegen, die einen direkten Vergleich mit medikamentösen Ansätzen erschweren.
