Behandlung eines Glomus caroticum mit perkutaner Embolisation und Ultraschallschere

Zusammenfassung

Paragangliome sind hoch vaskularisierte, meist benigne Tumoren, die aus Glomera sympathoadrenaler und parasympathischer Paraganglien des autonomen Nervensystems entstehen. Diese Tumoren werden in der Regel nach vorgängiger transarterieller Embolisation chirurgisch reseziert. Ziel dieser Studie ist die Beschreibung einer neuen Strategie, welche eine intraläsionelle perkutane Embolisation mit einer chirurgischen Dissektion per Ultraschallschere kombiniert. Hier wird der Fall einer 74-jährigen Frau mit einem Glomus-caroticum-Tumor des Typs Shamblin III dargestellt. Der kombinierte Einsatz von perkutaner Embolisation und Ultraschallschere erlaubte eine vollständige Resektion mit Erhalt der A. carotis interna und externa ohne postoperative Hirnnervenparese und mit minimalem Blutverlust. Die präoperative intraläsionale Embolisation mit einem flüssigen Embolisationsmittel weniger als 24 h vor der Op. in Kombination mit der Ultraschallschere scheint eine sehr gute Strategie in der Behandlung des Glomus caroticum zu sein.
