Migräne im Kindes- und Jugendalter – Gehirn und Muskel?

Warum Kinder einmal mehr keine kleinen Erwachsenen sind

Zusammenfassung

Die Migräne ist als primäre Kopfschmerzerkrankung eine für die Lebensqualität relevante Lifespan-Erkrankung auf dem Boden einer komplexen Genetik. Häufig beginnt sie schon im Kindesalter. Die Migräne zeigt alterstypische Symptome, Nuancierungen und Vorläufer, die sie deutlich vom späteren Lebensalter unterscheiden – so etwa in Bezug auf die Lokalisation der akuten Schmerzen. Erst im Laufe von Jahren – in Adoleszenz und jungem Erwachsenenalter – entwickelt sich die für die Migräne definitionsgebende, in der Klassifikation der International Classification of Headache Disorders (ICHD-3 beta) hinterlegte, Eindeutigkeit. Wir fokussieren in diesem Beitrag auf Kinder und Jugendliche mit Migräne. Dabei widmen wir uns ausführlicher dem trigeminozervikalen Komplex (TCC), weil er eine konzeptionelle Brücke zum Krankheitsverständnis der Migräne, zur Psychoedukation und zur Therapie bildet. Therapiemöglichkeiten und Therapiegrenzen werden pragmatisch diskutiert.
