Zusammenfassung

Die koronare Herzkrankheit (KHK) und ihre Folgeerkrankungen führen nach wie vor zu den meisten Erkrankungs- und Todesfällen in Deutschland. Die kardiale Computertomographie (CT) und Magnetresonanztomographie (MRT) erlauben eine präzise diagnostische Klassifikation und Risikostratifikation von Patienten mit Verdacht auf KHK oder Progression einer bekannten KHK. Die beiden Methoden sind nicht additiv und sollten u. a. mit dem klaren Ziel eingesetzt werden unnötige diagnostische Eingriffe mittels invasiver Herzkatheterdiagnostik zu vermeiden. Somit können die 2 Methoden als „Gatekeeper“ invasiver Verfahren dienen und sicherstellen, dass nur die Patient:innen einer invasiven Herzkatheterdiagnostik zugeführt werden, die diese wirklich im Sinne einer Koronarintervention benötigen. In diesem Zusammenhang ist der sachgerechte Einsatz dieser Bildgebungsmodalitäten durch die Fachdisziplin Kardiologie entscheidend, die über grundlegende Kenntnisse und die notwendige klinische Expertise in der Interpretation der klinischen Symptomatik und der Erfassung der Vortestwahrscheinlichkeit verfügt. Durch die Fachdisziplin Kardiologie werden Alter, Geschlecht und typische vs. atypische Angina pectoris berücksichtigt und wesentliche Komorbiditäten in Hinsicht auf die Planung der Diagnostik und Therapie im gesamtinternistischen Kontext gewertet. Die Kenntnis der Koronarangiographie ist entscheidend, um die Befunde der kardialen CT korrekt zu interpretieren und für die weitere Diagnostik und Therapie patienten- und nicht methodenzentriert zu bewerten. Nur durch eine bedarfs- und leitliniengerechte Anwendung der kardialen CT und MRT lässt sich eine Mengenausweitung der beiden Verfahren vermeiden, die unser Gesundheitssystem signifikant belasten würde. Im folgenden Positionspapier wird deswegen die bedarfs- und leitliniengerechte diagnostische und therapeutische Abfolge bei Abklärung mittels kardialer CT und MRT je nach klinischer Wahrscheinlichkeit für eine relevante obstruktive KHK aufgeführt.
