Korrektur der Nasenspitze durch endonasale Zugänge und Nähte

Zusammenfassung

Hintergrund

Während der letzten 2 Jahrzehnte wurden der externe Zugang und Transplantate nicht nur zur Nasenspitzenkorrektur zunehmend eingesetzt, sondern auch für weniger komplexe rhinochirurgische Probleme. Dieser Beitrag zeigt kasuistisch die Möglichkeiten des endonasalen Zugangs ohne Transplantate zur Nasenspitzenkorrektur.

Material und Methode

Retrospektiv wurden der Zugang zur Nasenspitze und die gewählte Technik für die Spitzenkorrektur bei 100 Rhinoplastiken untersucht.

Ergebnisse

In 81% der Fälle erfolgte die primäre oder die Revisionskorrektur der Nasenspitze endonasal, vorzugsweise über den infrakartilaginären Zugang. Durch die Kombination von Transfixion, Ankernähten, „Tongue-in-groove-Technik“ und Modifikationen der Flügelknorpel durch Nähte konnte i. d. R. auf Transplantate für die Spitze verzichtet werden.

Fazit

Die Korrektur der Form und Position der Nasenspitze über einen endonasalen Zugang unter Verwendung von Nähten ist technisch anspruchsvoller als der externe Zugang, geht aber mit kürzerer Op.-Dauer und Erholungszeit des Patienten einher. Entgegen dem allgemeinen Trend sollten die endonasalen Techniken der Nasenspitzenkorrektur mit Nähten fester Bestandteil des rhinochirurgischen Repertoires bleiben.
