Ultraschalldiagnostik der extrakraniellen hirnversorgenden Arterien

Zusammenfassung

Die farbkodierte Dopplersonographie (FKDS) ist das primäre angiologische Verfahren bei den meisten klinischen zerebrovakulären Fragestellungen. Der Ultraschall ist nicht-invasiv, kostengünstig und beliebig oft wiederholbar. Kein anderes vaskuläres Verfahren bietet eine so hohe örtliche und zeitliche Auflösung wie die simultane Akquisition morphologischer B‑Bild-Informationen und hämodynamischer Befunde mittels Dopplersonographie. Der korrekte, reproduzierbare Einsatz der FKDS erfordert Kenntnis technischer Grundlagen und Geräteparameter. Die klinisch hoch relevante Detektion und korrekte Quantifizierung einer Abgangsstenose der A. carotis interna (ACI) sind mit der FKDS bei Verwendung multiparametrischer Kriterien zuverlässig durchführbar. Wegen der schwierigeren Beschallung der A. vertebralis erfolgt die Stenosequantifizierung hier semiquantitativ. Die FKDS hat zentrale Bedeutung beim Monitoring von Eingriffen sowie bei der Verlaufsbeurteilung dissektionsbedingter Stenosen und Verschlüsse.
