21. Kultivierte autologe Hautzelltransplantate: Historie, Regulativa und Praxis

Zusammenfassung

Schwerbrandverletzte mit verbrannter Körperoberfläche >60 % bedürfen aufgrund des unterschiedlichen Läsionsmusters eines individuellen Behandlungsplans. Dabei sind mögliche Spalthautentnahmestellen sowie Mehrfachentnahmen einzubeziehen. Angesichts des Dilemmas einer möglichst geringen Expansion der Hauttransplantate und der limitierten Fläche an möglichen »donor sites« besteht Bedarf an in vitro gezüchteten Keratinozyten. Kultivierte autologe Hautzelltransplantate können bei ausgedehnten thermischen Wunden der Grade IIb und/oder III eine notwendige therapeutische Option sein. Wenn die Fläche gesunder Haut nicht ausreichend für eine adäquate Deckung mittels »Autografting« ist, kann eine Vervielfachung der transplantierbaren Fläche erforderlich werden. Neben perioperativen Verfahren wie Mesh- und Meek-Technik ist die autologe Zellkultur die einzige Möglichkeit, die mittels »Autografting« zu deckende Fläche erheblich zu erhöhen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
