Zusammenfassung

Hintergrund und Ziel der Arbeit

Trotz des hohen technischen Anspruchs werden laparoskopische Operationsverfahren auch in der Pankreaschirurgie zunehmend eingesetzt. In diesem Übersichtsartikel soll versucht werden, vor dem Hintergrund der landesweit zu hohen Mortalität in der Pankreaschirurgie und der bisher unklaren onkologischen Validierung die Wertigkeit dieser Verfahren anhand der publizierten Literatur zu bewerten.

Material und Methoden

In den letzten Jahren veröffentlichte retrospektive Kohorten- und Registerstudien untersuchten sowohl perioperative Ergebnisse als auch das Langezeitüberleben nach onkologischer laparoskopischer Pankreasresektion.

Ergebnisse und Diskussion

Laparoskopische Eingriffe am Pankreas werden zunehmend zur Behandlungen maligner Tumoren des Pankreas eingesetzt. Hierbei konnten die Vorteile der Laparoskopie, wie ein geringerer intraoperativer Blutverlust, eine Reduktion des postoperativen Schmerzes und eine Verkürzung des Krankenhausaufenthalts, in retrospektiven Studien gezeigt werden. Auch eine dem offenen Vorgehen äquivalente onkologische Therapie konnte gezeigt werden. Allerdings zeigte sich auch, dass die Mortalität bei laparoskopischen Eingriffen am Pankreas in Zentren mit geringer Fallzahl signifikant erhöht ist. Prospektive Studien, welche eine ausreichende onkologische Ergebnisqualität beweisen, sind weiterhin dringend erforderlich.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
