32. Laparoskopische Sigmaresektion bei Divertikulitis

Zusammenfassung

Die laparoskopische Sigmakontinuitätsresektion stellt das Standardverfahren für die Behandlung der Divertikulitis dar. Sie gilt als klassische Einsteigeroperation für die kolorektale minimalinvasive Chirurgie. Indiziert ist der Zugangsweg v. a. in der Elektivsituation bei chronisch rezidivierenden bzw. chronisch komplizierten Stadien. In der Notfallsituation entscheidet die Expertise des Operateurs über die Wahl des Zugangsweges. Entscheidend bei der laparoskopischen Resektion ist die Mitnahme des rektosigmoidalen Übergangs. Eine Entfernung aller Divertikel oral des Entzündungstumors ist nicht erforderlich. Dieses Kapitel stellt die präoperative Diagnostik, die Indikationsstellung, das perioperative Management und technische Vorgehen bei der laparoskopischen Sigmaresektion sowie die potenziellen intraoperativen Komplikationen vor.

Bei älteren, multimorbiden Menschen mit akuter Cholezystitis wird eine Operation häufig nicht erwogen. Eine Studie aus Philadelphia zeigt nun jedoch, dass auch diese Patientinnen und Patienten von einer chirurgischen Therapie profitieren können.
