Zusammenfassung

Obere Sprunggelenk- (OSG-)Distorsionen weisen aufgrund ihrer Häufigkeit eine große klinische und auch volkswirtschaftliche Bedeutung auf. Es gilt, die einfache OSG-Distorsion mit einer folgenlosen Ausheilung von der potenziell komplizierten Distorsion mit Übergang in eine chronische OSG-Instabilität zu unterscheiden. Therapeutisch steht bei der akuten OSG-Distorsion ohne operativ bedürftige Begleitverletzung und auch bei chronischen Instabilitäten die konservative Therapie im Vordergrund. Bei Versagen der konservativen Therapie können durch anatomische Bandrekonstruktionen gute Erfolge erzielt werden. Arthroskopische Techniken bieten den Vorteil der Gelenkinspektion und Adressierung intraartikulärer Pathologien in Kombination mit einer Bandplastik. Begleitpathologien gilt es während des operativen Eingriffes adäquat zu adressieren, um persistierende OSG-Beschwerden zu verhindern. Bei Verdacht auf Syndesmoseninsuffizienz und tibiofibulare Instabilität sollten eine frühzeitige Diagnostik mittels Magnetresonanztomographie und eine operative Versorgung angestrebt werden.
