Knobe et al. [1] wrote an exciting paper about skin oxygen saturation predicting revision after operation in calcaneal fractures. In this diagnostic test, the author used white light spectrophotometry on the patients receiving open reduction and internal fixation for calcaneal fractures. They chose five locations to measure along the planned incision of an extended lateral approach to the calcaneus. At last, the authors find patients with at least one measurement below 20.5% were at risk for wound revision surgery (sensitivity 100%, specificity 48.5%). It is an essential step that uses spectrophotometry to predict incision healing [1, 2]. However, some critical issues need to be clarified in the methods. …
