Ligamentäre Kniegelenkverletzungen im Wachstumsalter

Zusammenfassung

Durch vermehrte sportliche Aktivität nehmen Verletzungen des Kniegelenks im Wachstumsalter zu. Die anatomische Situation mit den noch offenen Wachstumsfugen erfordert ein besonderes Vorgehen bei der Behandlung der häufigen Verletzungen im Wachstumsalter: vordere Kreuzbandruptur, Patellaluxation und Meniskusverletzungen. Die klinischen Ergebnisse der konservativen Therapie bei vorderen Kreuzbandverletzungen im Wachstumsalter sind der operativen Versorgung unterlegen. Die transepiphysäre Rekonstruktion ist mit guten klinischen Ergebnisse sicher durchführbar. Die Therapie der Patellaluxation im Wachstumsalter zeigte bisher unbefriedigende Ergebnisse. Neue Techniken bieten die Möglichkeit, das mediale patellofemorale Ligament anatomisch zu rekonstruieren und das Alignement zu verbessern. Isolierte Meniskusverletzungen sind selten; der Scheibenmeniskus stellt eine anatomische Besonderheit dar. Sofern möglich, empfiehlt es sich, den Meniskus durch Naht zu erhalten. Ein Scheibenmeniskus sollte zurückhaltend reseziert und anatomisch rekonstruiert werden.
