Zusammenfassung
Das akute Leberversagen (ALV) wird im Erwachsenenalter bei einer schweren Lebererkrankung eines zuvor Lebergesunden über das Auftreten einer hepatischen Enzephalopathie (HE) innerhalb von 8 Wochen definiert. Im Gegensatz dazu beruht die Definition im Kindesalter, unabhängig vom Auftreten einer HE, ausschließlich auf einer schweren Leberfunktionsstörung, gekennzeichnet durch eine schwere Synthesestörung (Quicktest <40 %, Cholinesteraseaktivität <2,5 kU/l vor Substitution von FFP [„fresh frozen plasma“]), in der Regel in Assoziation mit einer konjugierten Hyperbilirubinämie und einer Transaminasenerhöhung. Eine gering- bis mäßiggradige HE ist dabei vor allem im Säuglingsalter kaum zu diagnostizieren. Während puristisch betrachtet die Bezeichnung ALV ausschließlich für zuvor lebergesunde Patienten verwandt werden dürfte, wird sie klinisch in der Regel auch auf Patienten ohne vorher bekannte Lebererkrankung ausgedehnt, so dass auch eine akute Dekompensation („acute-on-chronic liver failure“) einer vorbestehenden, jedoch nicht diagnostizierten Lebererkrankung (z. B. Morbus Wilson) eingeschlossen ist. Das pädiatrische ALV (pALV) ist selten und war unter rein supportiven Maßnahmen mit einer extrem schlechten Prognose (Letalität 70–>95 %) vergesellschaftet. Die Versorgung dieser Patienten in hierauf hoch spezialisierten pädiatrischen Zentren (mit allen therapeutischen Möglichkeiten, einschließlich einer pädiatrischen Lebertransplantation [pLTx]) ist die unabdingbare Voraussetzung dafür, dass aktuell Überlebensraten von 70 bis knapp 90 % erreicht werden können. In den großen pädiatrischen Lebertransplantationszentren machen die pALV einheitlich etwa 10–15 % der Indikationen zur pLTx aus.
