7. Psycholog ische Mechanismen der Chronifizierung – Konsequenzen für die Prävention

Zusammenfassung

Zahlreichen psychologischen und psychobiologischen Merkmalen kommt eine bedeutende Rolle als Risikofaktoren im Prozess der Chronifizierung akuter Schmerzen zu. Hierzu zählen eine depressive Stimmungslage, ungünstige Formen der emotionalen, kognitiven und verhaltensbezogenen Schmerzverarbeitung sowie chronische Stressoren im beruflichen und privaten Alltagsleben. Untersuchungen zu vermuteten Mechanismen umfassen Prozesse der klassischen und operanten Konditionierung, kognitiver Aufmerksamkeitssteuerung, körperlicher Aktivität, psychobiologischer Wechselwirkungen sowie iatrogene Prozesse, die im Rahmen der medizinischen Behandlung von Schmerzpatienten eine Chronifizierung begünstigen. Leitlinien zur Behandlung akuter Schmerzen sehen als Konsequenz eine frühzeitige Diagnostik psychologischer Risikofaktoren (»yellow flags«) vor, deren Berücksichtigung zur Prävention der Schmerzchronifizierung beitragen soll.
