Zusammenfassung

Dermatologische Erkrankungen des Anorektums gehören mit zu den häufigsten Beschwerdebildern, mit denen der Proktologe, insbesondere in der Praxis, konfrontiert wird. Häufig ist Juckreiz das Hauptsymptom, das Analekzeme unterschiedlichster Genese auslösen können. Hier ist die Kenntnis der verschiedenen Differenzialdiagnosen entscheidend für die erfolgreiche Therapie. Es muss das Ekzem von Dermatosen und den diversen Infektionen abgegrenzt werden, wobei durch Geschlechtsverkehr übertragene Erkrankungen eine nicht unwesentliche Rolle spielen, wie Herpes-Infektionen oder auch die von humanen Papillomaviren induzierten Erkrankungen wie Condylomata acuminata. Die meisten dermatologischen Erkrankungen des Anorektums sind einer konservativen topischen oder auch systemischen Therapie zugänglich. Ausnahmen bilden die intraepithelialen Neoplasien, Malignome und jene Krankheiten, die unter dem Oberbegriff perianale/perineale Abszesse subsumiert werden können, wie die Akne inversa oder der Pilonidalsinus.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
