Summary
The report deals with the protracted course of sublethal cyanide poisoning in a 14-year-old boy (maximum HCN concentration in blood 105 μg%), which was diagnosed approximately 1 day after the intake of the poison. By then brain damage due to the poisoning which was complicated by cardiac arrest 4 hours within the exposure to the poison, was so severe that cerebral death could no longer be prevented. The onset of cerebral death was insidious defying the exact definition of the time of brain death. The patient stayed “alive” for about 3 days after the cerebral death. While the cerebral functions had ceased permanently, some spinal functions returned after a phase largely characterized by the absence of reactions (spinal shock). In particular, vigorous spinal motor reactions, blood pressure regulation on a spinal cord level, and increases in body temperature were observed. During postmortem examination signs of intravital cerebral death were found: non-reactive necrosis of the brain accompanied by softening of the upper cervical cord segments, pituitary anterior lobe necrosis, and displacement of cerebellar tissue fragments into the spinal canal.
Zusammenfassung
Wir berichten über eine protrahiert verlaufende subletale Cyanid-intoxikation bei einem 14jährigen Jungen (höchste HCN-Konzentration im Blut 105 μg%), die etwa 1 Tag nach der Giftaufnahme diagnostiziert wird. Zu diesem Zeitpunkt hat die Intoxikation, die durch einen Herzstillstand 4 Std nach Einnahme des Giftes kompliziert wird, schon zu einer so schweren Hirnschädigung geführt, daß sich der cerebrale Tod nicht mehr aufhalten läßt. Der Eintritt des Hirntodes ist schleichend und läßt sich nicht exakt datieren; er wird etwa 3 Tage “überlebt”. Während die cerebralen Funktionen bleibend erlöschen, kehren einige spinale Funktionen nach einer Phase weitgehender Reaktionslosigkeit (spinaler Schock) wieder. Insbesondere sind eine lebhafte spinale Motorik, eine Blutdruckregulation auf Rückenmarksebene sowie Anstiege der Körpertemperatur zu beobachten. Bei der Sektion finden sich die Zeichen des intravital eingetretenen Hirntodes: areaktive Nekrose des Gehirns, begleitet von einer Erweichung der oberen Halsmarksegmente, einer Hypophysenvorderlappennekrose und einer Verlagerung von Kleinhirngewebetrümmern in den Rückenmarkskanal.
