Summary
3 patients with seesaw nystagmus after severe head traumas are described.
The nystagmus is characterized by vertical opposed pendular eye movements. In addition to these vertical movements there are rotations of the eyes: the eye moving up intorts and the eye moving down extorts.
The nystagmic eye movements are analysed by electromyography, electronystagmography, and cinematography.
A myographic study showes that the nystagmic eye movements are caused by the vertical motors, as well by the recti as by the obliqui. It is demonstrated by nystagmography that the amplitude of the nystagmus is reduced when the patient's eyes are closed or when he is tired. The nystagmus is also decreased after giving Diazepam (Valium-„Roche“) to the patient.
The 3 patients had a bitemporal hemianopia. In addition 2 of them had a diabetes insipidus and irregularities in the production of sexual hormones. There were no signs of cerebral tumors.
It is assumed that the cause of the seesaw nystagmus is a lesion in the region of the chiasma and of the diencephalon.
Zusammenfassung
Es werden 3 Patienten beschrieben, bei denen nach schweren Schädelhirntraumen ein Schaukel-Nystagmus (seesaw nystagmus) auftrat.
Der Nystagmus ist durch vertikale Pendelbewegungen charakterisiert, die an beiden Augen gegensinnig verlaufen. Das jeweils nach oben bewegte Auge wird um seine sagittale Achse einwärts, das nach unten bewegte Auge auswärts gerollt.
Die Augenbewegungen wurden mit Hilfe der Elektromyographie, Nystagmographie und Kinematographie analysiert.
Myographisch wurde nachgewiesen, daß sowohl die geraden als auch die schrägen Vertikalmotoren die Nystagmusbewegungen hervorrufen. Nystagmographisch zeigte sich, daß der Nystagmus durch Lidschluß und Ermüdung gehemmt wird. Auch Diazepam (Valium-„Roche“®) dämpft den Nystagmus.
Bei allen 3 Patienten bestand eine bitemporale Hemianopie. Zwei von ihnen hatten einen Diabetes insipidus und Sexualhormonstörungen. Für einen Tumor fand sich kein Anhalt.
Als Ursache des Schaukel-Nystagmus wird eine Strukturschädigung im Gebiet des Chiasma und des Diencephalon angenommen.
