Summary
Mastodynia has previously been treated with gestagens or gestagenbased ovulation inhibitors with only marginal success. No other satisfactory therapy was available and in the search for a better treatment, the effectiveness of long term administration of the prolactin inhibitor bromocryptine (CB 154) to 15 patients was evaluated. Five of the subjects exhibited mammary secretion as well as mastodynia which, according to palpatorial, cytological and X-ray criteria, was not caused by intraductal pathology. After two to four weeks treatment with 5 mg CB 154 per day ten patients recovered fully, three showed some improvement and two were totally resistant to the treatment. Plasma prolactin levels during the follicular stage measured prior to treatment were in the normal range. All the patients continued to ovulate during the course of treatment despite the irrefutable fact that prolactin release from the pituitary was inhibited. Since there was a similar inhibition of prolactin secretion in the two patients who were resistant to treatment, it would seem that prolactin though probably very important, cannot be the only decisive factor in the hormonal control of mastodynia. Further observations showed that the premenstrual syndrome can also be successfully treated with CB 154. Upon withdrawal of treatment the possibility or relapse must be considered.
Zusammenfassung
Die bisher übliche Mastodyniebehandlung mit Gestagenen oder gestagen-betonten Ovulationshemmern weist nur eine begrenzte Wirksamkeit auf. Da das Prolaktin auch auf das Mammagewebe nichtschwangerer Frauen einen regulativen Effekt ausübt, bot sich auf der Suche nach therapeutischen Alternativen die Überprüfung prolaktin-inhibierender Substanzen an. Zunächst wurde die Wirkung des Prolaktinhemmers Bromokryptin (CB 154, Sandoz AG) an 15 Patientinnen mit schwerer, bis dahin therapieresistenter Mastodynie getestet. 5 der untersuchten Patientinnen wiesen neben der Mastodynie eine Mammasekretion auf, die aufgrund palpatorischer, zytologischer und röntgenologischer Kriterien nicht als Folge eines pathologischen intraduktalen Prozesses anzusehen war. Nach zwei- bis vierwöchiger Behandlung mit täglich 5 mg CB 154 waren 10 Patientinnen völlig beschwerdefrei, 3 wiesen eine gewisse Besserung der Symptomatik auf. Lediglich 2 Patientinnen waren völlig therapieresistent. Hier zeigte sich auch keine Besserung der Mammasekretion.
Die vor Behandlungsbeginn während der Follikelphase analysierte Plasma-Prolaktin-Konzentration lag bei den untersuchten Testpersonen im Bereich der Norm. Trotz medikamentöser Prolaktin-Hemmung wiesen alle Patientinnen, die vor Behandlungsbeginn einen ovulatorischen Zyklus besaßen, weiterhin während der dreimonatigen Therapiedauer einen biphasischen Basaltemperaturverlauf auf. Da auch bei den therapieresistenten Fällen eine eindeutige Hemmung der hypophysären Prolaktin-Freisetzung nachweisbar war, ist das genannte Hormon wohl bedeutsam, aber nicht allein entscheidend für die Aufrechterhaltung der Mastodynie. Aus der Untersuchungsserie geht weiterhin hervor, daß das praemenstruelle Syndrom durch eine Bromokryptin-Therapie günstig zu beeinflussen ist. Nach Beendigung der dreimonatigen Behandlung kam es in allen Fällen zu einem Wiederauftreten der Mastodynie bzw. zu einem Rezidiv des praemenstruellen Syndroms.
