Zusammenfassung
An 25 jugendlichen Lippen-Kiefer-Gaumen-Spaltpatienten wurden die Auswirkungen der Cheiloplastik auf die dentofaziale Morphologie untersucht. Dazu wurden folgende Messungen durchgeführt: a) Oberlippenkraftmessung, b) elektromyographische Aktivitätsbestimmung der Lippenmuskulatur, c) röntgenkephalometrische und Weichteilanalyse im Fernröntgenseitenbild, d) Modellanalyse. Die Befunde wurden denen von 15 Nichtspaltträgern mit normaler Okklusion und Funktion gegenübergestellt. Nur zwischen der Ruhekraft der operierten Oberlippe und dem sagittalen und vertikalen Gesichtsschädelaufbau bestand bei den Lippen-Kiefer-Gaumen-Spaltpatienten ein positiver Zusammenhang, der auf die veränderte funktionnell-mechanische Wirkung der perioralen Muskulatur bei diesen Patienten zurückgeführt wird. Die Kraftwirkung der operierten Oberlippe auf ihre knöcherne Unterlage wurde durch verschiedene Faktoren beeinflußt: a) durch die Ruheaktivität der Oberlippenmuskulatur und b) durch die Belastungsrichtung der Kraft, die durch die Länge und Dicke der Weichteile bestimmt ist. Diese Befunde waren spezifisch für Patienten mit Lippen-Kiefer-Gaumen-Spalten und zeigten keine Alters- und Geschlechtsabhängigkeit. Im Gegensatz zu den dentalen Fehlstellungen konnten durch die kieferorthopädische Behandlung die skelettalen Veränderungen als Folge der muskulären Störung nicht ausgeglichen werden. Die Ergebnisse belegen, daß die richtige Wahl der Lippenplastik sowohl für die Gesichtsschädelentwicklung bei Lippen-Kiefer-Gaumen-Spaltträgern als auch für die Möglichkeit ihrer kieferorthopädischen Behandlung sehr wichtig ist. Aus theoretischen Überlegungen wird unter funktionell-mechanischen Gesichtspunkten der Lippenplastik nachDelaire der Vorzug gegeben.
Summary
The effect of primary lip repair on the dentofacial morphology was analysed in 25 adolescents with cleft lip and palate by means of a) upper lip pressure measurement, b) electromyography of the labial musculature, c) roentgenocephalometric and soft tissue analysis of the lateral head plate, and d) plaster model analysis. The results were compared with 15 patients presenting with normal occlusal and functional relationships. Only the resting pressure of the upper lip correlated with the vertical and sagittal dentofacial morphology in patients with cleft lip and palate. This relation is attributed to the unbalanced and unfavourable functional-mechanical strain of the perioral muscles in these cases. The strain of the repaired upper lip on the underlying skeletal structures correlated with a) the resting activity of the upper lip and b) the strain direction being determined by the length and thickness of the labial soft tissue. These results were specific for patients with cleft lip and palate and were not correlated to the patient's age and sex. Contrary to the dental parameters, the skeletal disturbances as a consequence of the muscular imbalance, could not be significantly altered by orthodontic treatment. The choice of technique of primary upper lip repair in patients with cleft lip and palate is important both for their dentofacial development and for their possible orthodontic treatment result. Theoretically, from the functional-mechanical aspect, our results indicateDelaire's method of primary functional upper lip repair to be the most suitable technique.
Résumé
Chez 25 adolescents atteints de fentes labio-maxillaires on a analysé l'effet de la chéiloplastie primaire sur le développement dento-facial. L'étude est basée sur a) la mesure de pression de la lèvre supérieure, b) l'électromyographie de la musculature labiale, c) l'examen céphalométrique du squelette facial et des tissus mous à l'aide de la téléradiographie de profil et d) l'analyse des moulages. Les résultats ont été comparés avec 15 patients ayant des relations occlusales et fonctionnelles normales. Les résultats montrent que seulement la pression de la lèvre supérieure en position de repos est en corrélation avec la morphologie dento-faciale dans la dimension verticale ainsi que dans la dimension sagittale; des relations qui ne se manifestent que dans les cas atteints de fentes labio-maxillaires. Les résultats sont attribués à l'altération fonctionnelle-mécanique du vecteur de force musculaire périorale. L'influence de la lèvre supérieure sur les structures osseuses sous-jacentes dépend: a) du tonus de la lévre supérieure en position de repos et b) de la direction de force qui est déterminée par la longueur et par l'épaisseur des tissus mous périoraux. Ces résultats sont spécifiques pour les adolescents atteints de fentes labio-maxillaires et sont indépendants de l'âge et du sexe. Au contraire des paramètres dentaires, les anomalies osseuses qui résultent de trouble musculaire ne peuvent pas être égalisées par les interventions orthodontiques. Le choix de la technique de chéiloplastie primaire pour fentes labio-maxillaires est aussi bien important pour le développement dentofacial que pour le résultat orthodontique qu'on peut atteindre. En théorie, ce qui concerne le point de vue fonctionnel-mécanique, les résultats indiquent que la chéilo-rhinoplastie primaire deDelaire doit être la technique avantageuse.
