‘There is but one safe way to avoid mistakes—to do nothing, or at least, to avoid doing something new. This, however, in itself, may be the greatest mistake of all.’
Summary
The literature on the subject of pathogenesis of oedema of the optic disc (Papilloedema) in intracranial space-occupying lesions and intracranial hypertension has been reviewed. The subject was investigated experimentally in detail in 77 rhesus monkeys and 26 rabbits. No oedema of the optic disc could be produced by acute intracranial hypertension in monkeys. With progressively growing experimental intracranial space-occupying lesion, situated in different regions of the brain, oedema of the optic disc was produced successfully in monkeys but not in rabbits. The various factors influencing the maximum size attained by the intracranial space-occupying lesion, the cerebrospinal fluid pressure (CSFP), and the fundus changes were investigated and co-related. A rise of CSFP in the cranial sub-arachnoid space and its successful transmission into the sheath of the optic nerve, with a rise of pressure in the latter, was found to be essential for the production of oedema of the optic disc. The degree of communication between the cranial cavity and the sheath of the optic nerve was investigated and was found to be variable in the region of the optic canal. The investigations excluded the role of compression of the central retinal vein, the extension of oedema of the brain, and the forcing of the cerebrospinal fluid from the sheath of the optic nerve along the perivascular space in the optic nerve in the production of oedema of the optic disc. Fluorescein fundus photographic studies revealed an increased capillary permeability in the optic disc in its oedema.
The various theories of pathogenesis of oedema of the optic disc described in the literature have been discussed in the light of the new facts that have emerged from the present experimental investigation.

L'auteur a fait un compte rendu de la littérature au sujet de la pathogénie de la stase papillaire (Papilloedema) en relation avec des lésions intracraniennes encombrantes et de l'hypertension intracranienne. Ce sujet a été examiné en détail par des expériences sur 77 singes Rhésus et 26 lapins. La stase papillaire n'était pas obtenue sur les singes par l'hypertension intracranienne aiguë. L'auteur a réussi à produire la stase papillaire sur des singes, mais pas sur des lapins, à l'aide de ballons intracraniens, qui s'agrandissaient progressivement et prenaient toujours davantage de place, dans des parties différentes du cerveau. Les points différents, qui influencent la grandeur maximale de la lésion intracranienne encombrante, la pression du liquide céphalo-rachidien (CSFP), et les changements au fond de l'oeil, ont été examinés et rapportés l'un à l'autre. L'auteur a trouvé qu'une augmentation de la pression du liquide céphalo-rachidien dans l'espace sous-arachnoïdien et sa transmission dans la gaine du nerf optique, avec une augmentation de la pression dans la dernière, est essentielle pour la production de la stase papillaire. Le degré de communication entre la cavité cranienne et la gaine du nerf optique a été examiné et l'auteur a trouvé qu'il est variable dans la région du canal optique. Les recherches de l'auteur ont exclu, dans la formation de la stase papillaire, les points suivants: le rôle de la compression de la veine centrale de la rétine, le prolongement de l'oedème du cerveau, et la pénétration forcé du liquide céphalorachidien de la gaine du nerf optique jusque dans l'espace périvasculaire dans le nerf optique. La photographie avec fluorescéine du fond de l'oeil a révélé une perméabilité capillaire agrandie dans le nerf optique oedémateux.
Les théories différentes trouvées dans la littérature sur la stase papillaire ont été evaluées sous l'angle des faits nouveaux mis en évidence par les recherches de l'auteur.
Zusammenfassung
Die Literatur hinsichtlich der Pathogenese der Stauungspapille (Papilloedema) im Fall platznehmender intrakranieller Schädigungen und intrakraniellem Hochdruck wurde zusammengefaßt. Dieses Thema wurde an Hand von 77 Rhesusaffen und 26 Kaninchen experimentell genau untersucht. Keine Stauungspapille wurde durch akuten intrakraniellen Hochdruck bei Affen hervorgerufen. In Affen, jedoch nicht in Kaninchen, ist es dem Verfasser gelungen, Stauungspapille durch experimentelle, progressiv wachsende, intrakranielle, platznehmende Schädigungen in verschiedenen Gehirnteilen herbeizuführen. Die verschiedenen Faktoren, die die Höchstgröße des Gehirnballons, den Cerebrospinalflüssigkeitsdruck (CSFP) und die Änderungen im Augenhintergrund beeinflussen, wurden untersucht und in Zusammenhang gebracht. Der Verfasser hat gefunden, daß das Steigen des Cerebrospinalflüssigkeitsdrucks im Subarachnoidalraum, und seine Übertragung in die Sehnervenhülle und das Steigen des Flüssigkeitsdrucks, für der Erzeugung der Stauungspapille unentbehrlich ist. Das Ausmaß der Verbindung zwischen der Schädelhöhle und der Sehnervenhülle wurde untersucht und der Verfasser hat festgestellt, daß es in der Gegend des optischen Kanal variabel ist. Die hier beschriebenen Untersuchungen haben bei der Erzeugung der Stauungspapille die folgenden Faktoren ausgeschlossen: die Rolle der Kompression der Vena centralis retinae, die Fortpflanzung des Gehirnödems, und das Vorwärtstreiben der Cerebrospinalflüssigkeit von der Sehnervenhülle in den perivasculären Raum im Sehnerv. Fluoresceinphotographien des Augenhintergrunds zeigten eine vergrößerte kapilläre Durchlässigkeit im Sehnervkopf bei Oedem.
Die verschiedenen Theorien der Pathogenese der Stauungspapille, die in der Literatur beschrieben sind, wurden im Licht der neuen Tatsachen, die aus den derzeitigen Versuchen hervorgegangen sind, eingehend besprochen.
